Incomplete Grade Forms MS Word Format
The policy concerning the granting of incomplete grades for courses has
changed. Effectively immediately, the student must request an incomplete
grade, download the form, and meet with the instructor to complete the form.
The form must have the student's signature, the instructor's signature,
and the Dean or Graduate Director's signature on it. The form is then
forwarded to the Registrar's Office where the grade of "I" is put into
the computer. This must be done before grades are due. Instructors can
not give incomplete grades any more.
