Declaration of Finances

All persons or agencies providing assistance to the applicant must complete this form. Each person or agency providing such assistance must provide a Statement and Bank Certification of available funds. The bank statement(s) must be on official bank letterhead showing the availability of funds. The Declaration of Finances must also be dated within six months of issuing the I-20; otherwise a new Declaration of Finances will be required.
