1. Do I have to receive the Meningitis and the Hepatitis B vaccine before attending school at Christian Brothers University? No, you do not have to receive the vaccine/s before attending Christian Brothers University. Christian Brothers University is required by state law to provide all new students with information about the diseases and the vaccines. The decision is yours. It may be helpful to discuss the vaccines with your healthcare provider. 2. What if I haven’t decided about the vaccine/s when I fill out my health form? You must sign the waiver indicating that you have read the information even if you decide to receive the vaccine/s at a later date. 3. When do I turn in my Health Form? As soon as possible after receiving your admission packet, but no later than the first day of classes. You may turn your health form in to Admissions or the Student Life office. 4. Do I still need to turn in a Health Form even if I do not live on campus? Yes, a Health Form (with immunization records and waiver sections completed) is required for all students entering Christian Brothers University. 5. What if I do not have records showing that I have received two doses of the MMR vaccine? It is very important that you check all sources for your records. If you attended a high school in Tennessee and graduated 1999 or later, you should have received the vaccine. Check with your private physician, local health department, or your school for documentation. If you have not received the second dose, the vaccine is available in Health Resources. If you do not have documentation, you are considered not immunized. If you have not received the second dose, the vaccine is available at your local Health Dept. and Health Resources. 3. Does the Certificate of Immunization have to be signed by a physician if I am attaching an acceptable form of documentation described on the certificate? No, a physician signature is not needed if you are attaching a permanent Tennessee Certificate of Immunization Form 2414, or comparable form from another state’s Health Department, or a signed medical record from your health care provider. Photocopies are acceptable, as originals will not be returned. |
