No, you do not have to receive the vaccine/s before attending Christian Brothers University. Christian Brothers University is required by state law to provide all new students with information about the diseases and the vaccines. The decision is yours. It is very important that you check all sources for your immunizations records. If you attended a high school in the state of Tennessee and graduated 1999 or later, you should have received the vaccine. Sources to check for documentation would include: your private physician, clinic, health department, high school, college, or military records. If you do not have documentation, you are considered not immunized. If you have not received the second dose, the vaccine is available at your local Health Dept. and Health Resources.

3. Does the Certificate of Immunization have to be signed by a physician if I am attaching an acceptable form of documentation described on the certificate?

No, a physician signature is not needed if you are attaching a permanent Tennessee Certificate of Immunization Form 2414, or comparable form from another state’s Health Department, or a signed medical record from your health care provider. Photocopies are acceptable, as originals will not be returned.
